The 2027 theme is "Listening with Purpose: Centering Voices That Shape Compassionate Care” This conference aims to bring together palliative and hospice social workers to explore the power of connection, resilience, and shared purpose. In a care landscape shaped by systemic inequities, shifting policies, and ongoing challenges in service delivery, this forum provides a space to reflect, learn, and grow together. Through expert-led sessions and engaging dialogue, we seek to empower participants and strengthen our collective commitment to equitable, compassionate, and person-centered care.

I had been a hospice social worker for 10 years and had recently made the transition to inpatient palliative care social work. CONTINUING EDUCATION

Continuing Education Information

SWHPN’s 2026 Annual Forum will be held as a live, virtual conference.  SWHPN will be using Zoom for its 2026 virtual forum. To receive CE credits, you’ll need to attend the sessions live, be logged in individually, and make sure your Zoom display name matches the name you used to register.

Zoom automatically tracks which sessions you attend and the number of minutes you’re present. These total minutes are then used to calculate your CE credits for the 2026 Annual Forum.

​

Social Work Hospice + Palliative Care Network (SWHPN), #1716, is approved as a provider for social work continuing education by the Association of Social Work Boards (ASWB) www.aswb.org, through the Approved Continuing Education (ACE) program. 

​

Social Work Hospice + Palliative Care Network aka SWHPN maintains responsibility for the program. ASWB Approval Period: 7/18/2023 – 7/18/2026. Social workers should contact their regulatory board to determine course approval for continuing education credits.

 

Social workers participating in this conference can receive 1 (one) continuing education clock hour PER each hour attended of educational sessions for this virtual event live.
